Health Forms
The Health Office has a number of important forms available to parents/guardians:
Health Information Exchange Consent
This health history information form must be completed by parents/guardians at the start of each school year for placement in student registration packets. The authorization expires at the end of each academic year and must be renewed annually. The form is also available in Spanish here.
Authorization for Release of Information
This form, once signed by the parent/guardian, authorizes the health office staff to communicate with the student's physician or health care provider. For a writeable Word Document, click here.
Medication Administration Form
This form must be filled out by the student's physician or health care provider before the Health Office staff is asked to administer medication during the school day. For a writeable Word Document, click here. The form is also available in Spanish here.
